What You Will Need
Adult
Applying for a New Passport - age 16 or over:
- Certified copy of birth certificate from State you were born (not the hospital)
- Passport Photos (can get them at FedEx Kinkos, UPS Store)
- Personal check or money order payable to U.S. Department of State for $130.00
- Personal check, money order, credit card, or cash (exact amount) to the Clerk of District Court for $35.00
- US Passport application DS-11
- Wyoming driver’s license with photo (if out of state, a second form of ID) - Temporary licenses are unacceptable
Minors
Applying for any new Passport or renewing a minors passport (minors passports cannot be renewed) - Under 16 years of age:
- Both parents must appear or provide certified court papers regarding custody OR Form DS-3053 - Statement of consent (absent parent) notarized and photocopy of front and back of absent parent’s driver’s license
- Minor’s certified copy of birth certificate from State minor was born (not the hospital)
- Passport Photo (can get them at FedEx Kinkos, UPS store)
- Personal check or money order payable to U.S. Department of State for $100.00
- Personal check, money order, credit card, or cash (exact amount) to the Clerk of District Court for $35.00
- US Passport application DS-11
- Wyoming driver’s licenses from both parents (if out of state, a second form of ID) - Temporary licenses are unacceptable
Renewal
Renewal of a U.S. Passport (adult) is allowed within 5 years of expiration. (If the passport has been expired over 5 years, you will need to apply with the Clerk of District Court using the
DS-11 form.)
- Use Form DS-82 for renewing a passport
- New passport photo
- Personal check or money order payable to U.S. Department of State for $130
- Your signature on this application does not need to be witnessed and you do not have to bring this application to the Clerk of District Court. This application is mailed by you.
- Application must be sent to the U.S. Department of State using trackable mail.
U.S. Passport Card
A U.S. Passport Card can be used for ground or cruise ship travel to Canada, Mexico, parts of the Caribbean and the Bahamas.
- A U.S. Passport Card CAN NOT be used to fly outside the United States.
- Issuance of a new passport card requires the same documents that a new passport requires.
- Fees for a Card:
- Adult: $35
- Minor (under 16 years of age): $15
Expedite
With the exception of a passport card, any of the above processes can be expedited for an additional $60 fee per application. Overnight delivery to and from the U.S. Passport Agency is also available for an additional fee.
Waiting Period
Issue times for obtaining a passport book or a passport card will vary. Please contact the Clerk of District Court or visit travel.state.gov for the most up to date issue times.
All forms are available in the Clerk of District Court’s office or online at the Travel Website.
All applications must be completed in BLACK INK and signed in the presence of an acceptance agent in the Clerk of District Court’s Office unless otherwise indicated.
